Thank you for choosing this facility for your training needs.  This web page has been created to ensure all paperwork is completed prior to your rotation cycle. 

HIPAA and VA Information Security Awareness Training

 

To complete VHA Privacy Policy and VA Information Security Awareness courses, log onto  https://www.ees-learning.net/librix/loginhtml.asp?v=librix *.  You will need to register by clicking on the First Time User button.  After completion of each course print the certificate.  Both certificates MUST be turned in to your service ADPAC on the first day of your rotation.  You WILL NOT receive computer codes if you do not turn in your certificates.  This is a yearly requirement. 

 

NOTE:  If you have completed the HIPPA training at your school, please provide a copy of the certificate.  You will not need to take this course at both locations.  You will still need to complete the VA Information Security Awareness Course.  

 

You must also read and sign the National Rules of Behavior which can be found in the forms table below.  Service ADPACs must have the signed form to obtain your computer codes.

 WOC Appointment Request Two weeks prior to the start of your rotation:  Submit to the VA Education Office (Address is listed below under Point of Contact).

Two weeks prior to start of your rotation:   Submit to the VA Education Office (Address is listed below under Point of Contact).
HIPAA Certificate First day of your rotation: Submit the certificate to your service ADPAC.  On-line training should be completed prior to start of rotation.
Information Security Certificate First day of your rotation: Submit the certificate to your service ADPAC.  On-line training should be completed prior to start of rotation.
National Rules Of Behavior First day of your rotation: Read, sign the last page and hand in only the signed last page to your service ADPAC.
Accepted Forms of Identification Upon arrival at the VA you will need to report to Human Resources to sign your paper work.  Following this, go to Police and Security Services to be finger printed and to obtain an ID badge.  Two acceptable forms of identification will be required.  You will also need a "Form 0711" which can be obtained in the Education Office before going to Police and Security  for your ID badge.
