Early during the hospital stay, the health care team will talk to the patient about his or her after hospitalization care needs, anticipated discharge date, and how to receive follow-up care. The medical center has recently added specific discharge appointments in order to improve the discharge process.
- Discharges will be scheduled to allow the patient and his/her family to specify a time which is convenient for them to arrange transportation home.
- The discharge appointment will allow patients to leave throughout the day to avoid the rush of everyone going home at the same time.
- It will also allow staff to discharge patients ahead of planned admissions and times of high admission demand.
- It will allow staff to prepare for the discharge, providing instructions, medicine, supplies and appointments when the patient is scheduled to leave.
If you are on a special diet, a dietitian will counsel you and your family on how to follow your diet at home.
